Abstract
External genital endometriosis remains one of the most significant diseases, occurring, according to current epidemiological estimates, in 6-10% of women of reproductive age. Severe forms of the disease involving the retrocervical region and intestine present the greatest challenges for diagnosis and surgical treatment, requiring a multidisciplinary approach and a careful balance between the radicality of the intervention and the preservation of reproductive function.
In this clinical case, an organ-preserving laparoscopic procedure was performed, including excision of endometriotic foci, bilateral cystectomy with enucleation of the endometrioma capsules, and adhesiolysis. A 43-year-old patient with severe external genital endometriosis included bilateral ovarian endometriomas up to 9 cm in diameter, retrocervical endometriosis, and involvement of the rectosigmoid colon.
