Abstract

The aim of the study – to develop a program for the prevention of severe genital prolapse in sanatorium conditions in patients at risk of its development based on a prognostic formula.
Materials and methods. We conducted a prospective cohort study in one group of women of late reproductive and perimenopausal age with stage I-II PTO (n = 42) with assessment of the results before and after intervention (course of preventive therapy of PTO) in the period 2023-2025 in the conditions of «Altay Castle» (resort town of Federal significance Belokurikha, Altay Region, Russian Federation). The study evaluated clinical and anamnestic data, echographic data, questionnaire results, and perineometry. The risk of developing severe PTO was calculated using a formula based on logistic regression. The Mann-Whitney nonparametric test was used to assess the dynamics of the indicators before and after therapy. The results are presented as the median (Me) and interquartile range (Q1-Q3).
Results and discussion. The results obtained in the study made it possible to create a program for the prevention of severe genital prolapse in patients at risk who have the initial manifestations of the disease. The developed prognostic formula for the risk of progression of pelvic organ prolapse, based on clinical and anamnestic data, allows for the timely identification of a cohort of women and the provision of early preventive therapy aimed at strengthening the pelvic floor muscles, which is available in sanatorium conditions (diet, gymnastics, walking, aqua aerobics, electromagnetic stimulation of the pelvic floor neuromuscular apparatus using the KAP-ELM-01 Andro-Gin device, and herbal medicine).
Conclusion. The proposed program for the prevention of severe genital prolapse in sanatorium conditions, based on a prognostic formula, allows for personalized management of patients at risk of progression, with an emphasis on modifying risk factors.
