ОПУХОЛЬ БРЕННЕРА БОЛЬШОГО РАЗМЕРА В ПОСТМЕНОПАУЗЕ: КЛИНИЧЕСКОЕ НАБЛЮДЕНИЕ РЕДКОГО НОВООБРАЗОВАНИЯ ЯИЧНИКА

Abstract

Brenner tumor is a rare epithelial ovarian neoplasm, accounting for about 1-2% of all solid tumors of this localization [1-3]. Diagnosis of these neoplasms presents significant difficulties due to the absence of pathognomonic clinical and echographic signs, which often leads to the final diagnosis being established only after histological examination of the removed specimen [1, 4, 5]. Particular concern is raised by large tumors detected in the postmenopausal period, requiring differential diagnosis with malignant processes [6-8].
The article presents a clinical observation of a 58-year-old patient with a Brenner tumor of the right ovary measuring 10 cm in diameter, detected against the background of long-term menopause (16 years) and severe somatic pathology (morbid obesity with BMI 54.6, uncontrolled arterial hypertension). A feature of the case was the combination of an ovarian mass with subserous uterine myoma, which created additional difficulties in preoperative data interpretation [4, 9]. Surgical treatment in the form of bilateral laparoscopic adnexectomy and myomectomy allowed radical removal of the neoplasm. The postoperative period was uneventful. Histological examination confirmed the diagnosis of benign Brenner tumor [10, 11].
This observation demonstrates the need to include Brenner tumor in the differential diagnostic search for solid and solid-cystic ovarian masses in postmenopausal women [2, 7, 12], and also emphasizes the importance of a radical surgical approach with mandatory morphological examination [1, 5, 6].
