Abstract

Pregnancy after a kidney transplant is no longer a rarity, and there is a significant amount of data supporting its feasibility with stable allograft function. Similarly, successful pregnancies have been reported after isolated liver transplantation. However, pregnancy in a patient with two transplanted organs (a kidney and a liver) presents an exceptionally challenging clinical scenario. The combination of two allografts multiplies the complex of potential risks associated with both changes in the pharmacokinetics of immunosuppressants, hemodynamic shifts, hypercoagulation, and the risk of mutual influence of complications from each of the organs.
This report presents a clinical case of successful management of pregnancy and delivery in a patient after combined kidney and liver transplantation, with a detailed description of pre-gravidary preparation, the course of gestation, the tactics of immunosuppressive therapy, monitoring methods, and the outcome for the mother and the newborn. The purpose of this description is to summarize practical experience and highlight the key principles of managing such a complex multidisciplinary pathology.
