Abstract
The aim of this work is to analyze the clinical and diagnostic features of eosinophilic esophagitis (EoE) in children based on our own observations and current literature data.
The article emphasizes that EoE is a chronic immune-mediated disease whose pathogenesis is realized within the framework of Th2 inflammation against a background of genetic predisposition and atopic status. Special attention is paid to the age-related evolution of symptoms: from non-specific extraesophageal manifestations in young children to the development of typical dysphagia and endoscopic signs of remodeling in adolescents. Clinical examples illustrate diagnostic challenges, which consist in the frequent masking of symptoms as respiratory tract pathology and gastroesophageal reflux, leading to significant delays in diagnosis verification.
The conclusion is drawn about the need for increased vigilance regarding EoE in all children with persistent symptoms of upper gastrointestinal tract involvement and refractory atopic diseases. The main practical result is the justification for the mandatory performance of esophagogastroduodenoscopy with multiple biopsies in this category of patients for timely diagnosis and prevention of fibrotic complications. Successful patient management requires a comprehensive multidisciplinary approach.
